I don't have the first page to the US data. Unlike all the other countries, it just shows 'this page intentionally blank'. I think there's supposed to be a page explaining the country's background.

I guess what I mean is the intro page for the US data. That page is missing in the PDF. Is it possible to get a copy of that page?
