Dave,
WRT your first question, yes in #3 you should use a D12 IAW the COLLISION DAMAGE table. Obvious typo in the rules. Going back, it looks like the table changed from a D6 to a D12 in Amendment 1. Update to the rule text was obviously missed.
WRT your second question, the bullet is stating how to deal with a sub collision since subs are not on the MINE & TORPEDO DAMAGE table. I suppose you'll need Lonnie, or someone similar, to confirm, but I believe the second sentence basically implies for a surface ship (vs. a sub), you assume the sub is in a column just right of the "DD - TB" column on the MINE & TORPEDO DAMAGE table, so essentially any surface ship at least TB sized is going to use the +1 column on the COLLISION DAMAGE table. It is somewhat unfortunate the sentence is in italics, indicating an example.

Yes, Dave Franklin is correct. During the final stages of preparing GQ 3.3, we spent some time streamlining and smoothing the procedures such as using a D12 for most activities in place of a D6. Inadvertently, we did not update the Section 1.3.3 text. I have updated the text with the changes indicated in red on a pdf for those who would like to update their files.
In addition during my review, I found that mine damage to submarines was not clearly indicated. Thus, I have changed the "Torpedo" column on the SUBMARINE DAMAGE table on Chart 9-J to now read "Mine or Torpedo." A pdf of the revised chart, now 9-L, will also be made available.
These changes will also be posted in the Bonus Files on the Forum for those that may not have accessed this post.

PS: You will note that "torpedo tubes" is repeated at the bottom of page 1 - 4 revised as well as at the top of page 1 - 5. Seems MicroSoft has revised their formating slightly, yet again.
