The Panzer V Panther Ausf. D w/Panzer IV Turret was never a combat capable vehicle. If you want to make one I'd recommend combining the stats from a PZIV (OM and OV) with the stats from a PzV (everything except OV and OM).
Comments about the vehicle:
The only confirmed case of a Panther with a Pzkw IV turret was a command vehicle, used by Army Heavy Tank Destroyer Detachment 503. It served on the Eastern Front in early/mid 1944.
The vehicle was cobbled together by bolting a turret salvaged from a knocked out Pzkw IV Ausfg H onto the plated over hull of an early model Bergepanther armoured recovery vehicle (ARV). The gun was a dummy and the turret was fixed in place, serving only to camouflage the command vehicle's specialist role.
As for the T-34/76 the only reference I can find to "up armored" as a mention that refurbished T-34/76 had 30mm plate welded on to glacis and turret front and sides. This caused additional performance issues with gears and engine due to extra weight.
T-34/76 stats : you can add +3 to the DV front and side but reduce the CC Pips by 2 and the Road Pips by one. This should give you something you can use on the tabletop.
